Art of Physical Therapy provides out-of-network care, meaning we do not file directly with your insurance company. After your appointment, you may file a health insurance claim yourself. Some insurance plans cover out-of-network services or may allow you to apply the service cost toward your out-of-network deductible. If you would like to file a claim, these are the steps to do so:
Request a Superbill from your physical therapist. This document provides vital information such as services received, the location of services, and the cost.
Fill out a Member Claim Form from your insurance company.
Submit the Superbill and the Member Claim Form to your insurance company.
